The toolkit available to HCO’s to accomplish this goal spans enterprise, mobile, web, and personal monitoring devices. These technologies focus on improving communication, education, and self-care while patients are out living their lives beyond the walls of a health care facility. At Chilmark Research, our efforts focus on delivery system adoption of these tools, beginning with federal mandate programs but extending to the broader goals of population-level health system reform.
